WHAT IS CASE MANAGEMENT

The case manager is an important core member of a care team. The case manager helps clients and their loved ones obtain services and resources needed so clients can remain independent after experiencing a life-changing injury or disability.  The case manager is the team member that creates a plan, coordinates care among different providers, assists with obtaining services, equipment, supplies, technology, transportation and resources, and helps navigate complex benefit systems. The case manager is a support and resource for both the client and the family.

The Case Management Society of America defines case management as:

A collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s health needs through communication and available resources to promote quality cost-effective outcomes.

Serenity Case Management follows the standards of care established by the Case Management Society of America, the leading membership association providing professional collaboration across the healthcare continuum to advocate for patients’ well-being and improved health outcomes.

If injured in a motor vehicle accident, the Michigan Auto No-fault system is a unique system allowing medical coverage for a person’s care recovery and rehabilitation. Clinical care management, such as provided by Ambrose Care Management, is a covered benefit under the auto no-fault benefits.

What should I consider when choosing a case manager?

There are a number of qualities to consider when choosing a case manager. This list is not inclusive, but provides a guideline for choosing a case manager.

  • Above all, you feel comfortable with the case manager.
  • The case manager has an updated and unencumbered license or credentials of his or her professional discipline.
  • The case manager has field experience working with people who have sustained catastrophic injuries.
  • The case management company is a legally formed company within the state the home office is located.
  •  The case manager is aware of and has policies that follows HIPAA and privacy laws to protect your personal health information.
  •  The case manager is knowledgeable about your state’s auto no-fault and worker’s compensation benefits and is willing to help you understand them.
  •  The case manager is knowledgeable about resources in the community in which you reside.
  •  The case manager has working relationships with doctors, psychologists, providers, equipment and supply companies, labs, pharmacies, and transportation service within your community.
  •  After meetings with the case manager, you feel informed and your questions are answered.
  •  You feel the case manager understands your needs, provided options and solutions, and is eager to help you.

Serenity Case Management exists to help clients who have been injured in an auto accident. We want our case management services to be proactive, compassionate, and intelligent. The services provided mean everything to our clients and those who think to use our company for their patients or their clients. When we care about what we do to a level that is in the realm of going above and beyond, everyone will take notice. ABOUT US

MICHELLE

My name is Michelle and I am a Nurse Case Manager for Serenity Case Management.  Before starting Serenity with my partner Karen, I worked as a unit nurse for medical/surgical, step-down progressive care, intensive car, and as a Bariatric Surgery Coordinator.  I have always been a caregiver who empathizes with all ages and types of people in need of help.  Meeting patients and their families on some of the hardest days of their lives brings me a profound sense of purpose.  The amazing thing about being a Nurse Case Manager is not only working with patients, families and providers but being a part of the healing process.  I love to get to know patients and provide everything I can to help them physically, mentally, and spiritually. I love my job because it is who I am even when I am not working and grateful for the opportunity to serve others.

KAREN

My name is Karen and I am a Nurse Case Manager at Serenity Case Management.  I graduated from Michigan State University with a Bachelor of Science in Nursing and before becoming a case manager I worked as a clinical nurse at Beaumont Royal Oak.  As a nurse case manager, I get to help people in difficult situations by advising and helping them figure out what kid of assistance and services they need while creating personalized care plans to coordinate all of their care after a motor vehicle accident.  My favorite part of being a case manager is getting to see my patients heal and grow after debilitating accidents.  I love that I get to advocate for my patients and work with them, their families, and all their providers to make sure they are getting the best care possible.  Being a nurse, my patient's well-being is always my #1 priority and here at Serenity our very first core value is "Patient First".

Nurse Case Management

 

Nurse case managers uses a head to toe strategy that will identify injuries you sustain in motor vehicle injuries and we work with you and your provider to insure that you reach maximum medical recovery as soon as possible. Serenity Case Management services are structured to bring additional health care resources to auto related injuries. Nurse case managers make sure that they facilitate all of the cares and needs the client have with all of the providers that are chosen for them in the initial assessment that we do as a head to toe assessment and also we determine what doctors will they need to see. And then we facilitate the transportation needs for our clients. We also facilitate the coordination of care between the providers and also we make sure that they get all of their diagnostics test done and their reports are given back to the doctors. We do write reports about how the client is and how they are progressing from injury to recovery.
